Course Overview
Whilst the RCA process may be well understood and embedded in many industries, aviation regulations are now increasingly mandating more formal and comprehensive responses to oversight and compliance monitoring results. Many regulators are now requiring organisations to become pro-active in the use of Root Cause Analysis techniques to determine underlying causes of nonconformity or process weakness and arrive at suitable corrective actions. This two-day workshop has been developed to address this requirement in a fully practical rather than theoretical manner drawing on experiences gained both within and outside of the aviation industry.
This workshop is designed to provide training and practical experience in a range of approaches for those who need to understand, manage and/or undertake Root Cause Analysis as an integral part of problem solving in response to reported incidents or audit findings relating to safety and/or quality in the aviation industry. It is designed to be highly participative with interactive trainer led discussions, utilising diverse and aviation related examples as an integral part of the training process.

Course Topics
The workshop follows a logical progression for the RCA & Problem-Solving process together with the presentation of internationally adopted approaches to the identification of Root Causes and the determination of suitable corrective actions. Subjects addressed include:
- Background and the need for a Structured Approach
- Roles and Responsibilities
- The RCA Process
- Problem Identification, Clarification and Definition
- Determination of Likely Root Causes
- Determination of Suitable Corrective Actions
- Implementation and Verification
- A selection of RCA Tools and Techniques
- Process Analysis, Data Collection and Display techniques
The workshop adopts a modular approach;
Module 1 introduces participants to the recommended process together with various problem-solving techniques, including the often adopted 5 ‘Whys’ approach. It also covers the essential pre-requisites for the adoption of an RCA strategy in an organization together with the resource and structures required to ensure successful implementation. It also covers the essential aspects of terminology associated with RCA and Correction & Corrective action and attempts to dispel the myths and misunderstandings that have grown up with RCA recognizing the wealth of knowledge and experience that exist in other industries.
Module 2 is used to demonstrate how these techniques are used in practical situations, following which their application in relation to an aviation scenario is demonstrated by means of a series of focused workshop activities undertaken by the participants under trainer guidance. The approaches to Corrective Action determination and implementation are fully considered.
Module 3 introduces techniques used to capture and display data that may be used to assist generally with problem solving activities in an organization, together with a final discussion on introducing RCA into an organization.

This workshop will be of benefit to Industry & Regulatory staff who need to promote or become actively involved with undertaking root cause analysis within their organisation. It will be of very significant benefit to Managers, Nominated Postholders, Quality, Safety, & Compliance Monitoring Managers, auditors and those who need to have the necessary competence to conduct effective Root Cause Analysis in response to reported concerns, customer complaints, audit findings, regulatory oversight issues etc.
The Workshop will also be of great benefit to regulators who need to both promote the adoption of RCA but also to establish that the approaches being used are effective, and others who need to understand and ensure that the audit and associated RCA process provides them with sufficient assurance of the adequacy and effectiveness of operations in relation to regulatory compliance.
Learning Objectives
After attending this course participants will be able to return to their organisations with not only theoretical knowledge but more importantly practical experience of having undertaken various Root Cause Analysis activities in the safe learning environment of the Workshop. They will be in a good position to train and lead others in the implementation of effective Root Cause Analyses throughout their organisation. In particular they should leave the workshop with the following abilities:
- To assist in the determination of the necessity for undertaking a Root Cause Analysis.
- To assist in the prioritisation of RCA activities.
- To determine the focus of the RCA process in relation to reported audit nonconformities, management system deficiencies or incidents.
- To practically undertake RCA activities throughout the organisation, working with various techniques as appropriate to specific RCA activities.
- To determine and encourage the involvement of others in the RCA process.
- To determine suitable corrective actions in response to identified root causes.
- To act as coordinators and facilitators for RCA activities and to act as ‘champions’ for the adoption of RCA throughout the organisation.
- To decide on the suitability of available technique(s) to be used by those undertaking RCA activities, and to facilitate company-wide adoption of preferred methodologies.
- To encourage and gain the commitment of senior management to, and provide support for, the adoption of RCA throughout the organisation together with the provision of necessary resources.
Hence, the workshop provides an overview of the RCA process together with comprehensive training in relation to the use of a selection of tools and techniques which may be used as an integral part of a Root Cause Analysis & problem-solving activity. The Workshop draws on experiences gained within the aviation industry together with approaches observed in a range of industries and cultures that have been used as an integral part of the response to audit & compliance monitoring activities, reported nonconformities, customer complaints, regulatory oversight findings etc. and acting in support of the achievement of operational and business objectives.
The two days are highly participative and promote a problem-solving methodology and a range of techniques that will be of benefit to those who need to ensure the adequacy of response to reported management system and operational deficiencies.
